For a prescription refill, please have your pharmacy fax your request to 905-494-6758. 

If you need to speak with myself, and there is no response on the number on my card, please contact your home care nurse.

If there is still no response, for non-urgent matters please call 905-494-2120 x32730 to leave a message. For urgent matters only (e.g. patient is in pain) please call 416-747-3400 x 34567, and then ask to speak with the "On Call Palliative Care Doctor".
